STRATHROY DISTRICT MINOR HOCKEY REGISTRATION 2023/24
U5 – U18
SDMHA’s goal is to have the bulk of our registrations done by June 30th, 2023 in order to determine the number of teams and coaches required for the upcoming season. If you have any questions, please contact your SDMHA Registrar or Executive for clarification.
PLEASE READ THROUGH ALL OF THE FOLLOWING INFORMATION - WITH REGISTRATION, YOU HAVE AGREED TO HAVE READ THROUGH IT!

**Please note that registration is best done on a computer - the links do not always appear on a cell phone. Any difficulties with the HCR must be addressed with the HCR directly**
REGISTRATION & PLAYER ACCEPTANCE
1. New players will need to create an HCR account. After you have created an account, a verification code will be emailed to you. Please check your junk folder as it may land there. Enter the verification code and follow the prompts to register your player(s). Select the player you wish to register and complete the registration process by answering the questions and selecting the add on options you wish to choose. Once the player's registration process is complete, you will have the option to add a second or subsequent player to your cart. You should register all of your players in this way before entering the payment process.
* Please note you may receive an error message when registering for U5 that says your child is too young. You should be able to skip past that warning message and proceed to registration. If you are unable to proceed, please email [email protected]
All new players to minor hockey are required to upload their birth certificate and proof of residency (photo of a parent's driver's license or utility bill that matches the registration address) to their profile. The system will allow you to complete the registration process without this but you will not be approved by OMHA until it is there.
All new players are also required to have a course completed for "Respect In Sport". If you have had other children in minor hockey, you should only need to add your new player to your existing account. Instructions for both new families and returning families can be found HERE
* Please note that RIS charges a small fee for the course.

Players are not considered registered until the online registration and waiver form are completed, AND a 20% deposit is received.
**If the required division reaches its limit, the player will be placed on a waiting list pending SDMHA's review of the division and registration numbers. Registration is on a first-come, first-served basis - returning SDMHA players are not guaranteed a spot if registration becomes full. Please be sure to register early!!

U5 (2019/2020) - $450
U6 (2018) - $550
U7 (2017) - $550
U8 (2016) - $850
U9 (2015) - $850
U10 (2014) - $1050
U11 (2013) - $1050
U12 (2012) - $1100
U13 (2011) - $1100
U14 (2010) - $1100
U15 (2009) - $1100
U16 (2008) - $1100
U18 (2006/2007) - $1100
**Please remember to add the Tryout Fee to your cart if your child is trying out for U10-U18 divisions only*

CASH CALENDAR - $150 (U6-U18 - sell your 15 tickets and make this money back)
**NON REFUNDABLE if a registration refund is requested**
VOLUNTEER - $200 (U8-U18 – sign up for shifts of time and you receive this back)
TRYOUT - $100 (applicable to U10 - U18 players only)
Travel Fees: $250 (this fee is only for players who make competitive teams and it is collected by the team once player is rostered).
FEE PAYMENT
Fees will be paid by credit card only through SportsPay - arrangements to be made online through the HCR.
PERMISSION TO SKATE (for players wishing to try out for AAA, AA ONLY!)

DRUG & ALCOHOL POLICY
U13 - U18 players and their parents will be required to read, sign and adhere to SDMHA’s Drug and Alcohol Policy, which can be found on our website in the Policy and Procedures Manual. Forms will be distributed to players once rostered.
REFUNDS
Refund policy can be found on the website at SDMHA.org.
CODE OF CONDUCT
All players and members are expected to follow the Code of Conduct Policy and the SDMHA Social Media Policy that can be found on our web site in the Policy and Procedures Manual.
PRIVACY OF INFORMATION
Your registration signifies you understand and agree that your child's personal information will be kept by SDMHA for the sole use of SDMHA and the OMHA over the course of this hockey season. Please contact SDMHA directly if you do not wish your child’s picture to be displayed on our team boards.
